2. 2 If you have a HealthPartners username and password, enter it here with your company name and that is your only step to take your health assessment.

3. 3 If you click register for an account on the first screenshot, you will see this screen next and need to enter your company name to continue.

4. 4 Then you will need to verify if you have HealthPartners insurance.

5. 5 If you click yes that you have HealthPartners insurance, this screen is step 1 of the registering process.

9. 9 Finally, new registrants must accept the conditions to continue.

10. 10 Along the way, if you enter an incorrect company name, you will be prompted to call customer service.
