Preparing for 5S All departments should be audited prior to 5S activity to establish baseline data. Prior to conducting 5S activities, all affected employees need to receive training and expectations for the 5S initiative. ►Review corporate 5S training powerpoint located on the LSS website. ►Establish plant-specific guidelines on acceptable items. ►The recommended auditor team method is three (3) people with cross-functional representation.

Opening the Excel form Always answer “Yes” when opening the audit form

Identify Audit Areas – Plant Specific List all departments as defined in your facility. Define/clarify physical locations as necessary. Each plant will need to adjust the “Department” and “Description” lists as necessary to ensure the drop-down menu functions correctly.

Start Audit All facilities must use the audit form to provide a consistent evaluation. Click here to start audit. Follow instructions to get started. 2 3 4 5

Answer “Yes” or “No” to each question. Scores are automatically tabulated When all questions are answered, proceed to next “S” by clicking here. Add any comments or suggestions. Auditing each “S”

Continue answering the questions for each “S”. Next level scores do not apply until all questions are answered “Yes” in the previous level. Continue through each of the 5S questionnaires: Sort Simplify Shine Standardize Sustain During Audit

After answering the questionnaire for all of the 5S categories, click “Finish”. Completing Audit

Clicking “Finish” will populate the radar chart to give you the final department score During Audit
