Who and where we are Official title: NAVIGATORS Bilingual team Work from harm reduction perspective considering not only the clients’ addictions needs but also: ▫Mental health ▫Housing ▫Primary care ▫Income support needs Located at OWMC with varied locations for walk-in clinics to be determined ▫1777 Montreal Road

What we do Screening for “fit-to-service” Provide information to potential clients and family members Brief/initial assessment to determine where the person should be referred to for appropriate service ▫Residential ▫Day ▫Community Referral planning to addictions services and other sectors ▫Not only in Ottawa, but other cities in the province too

Services we offer Walk-in clinics where we will provide information and conduct brief assessments Family information regarding addictions services and support services available to them in the community One-on-one services where appropriate (literacy or comprehension issues) Bridging

Bridging Supports Developed 3 community calendars ▫Addictions oriented ▫Health and well-being ▫Family supports Check-ins with clients and family members On-going contact with agencies for new programs and supports in the community
