Student Achievement Plan A Guide for Development

Objective: Teachers will analyze student data to drive instruction and increase student achievement.

Q.1 Year Long Goal: What year long goal will you be measuring and reporting on your students? What do you want your students to know and be able to do? Tips – Written as a goal – Good Goal Sample: I would like my students to improve their Math skills. I would like them to advance skills in the specific area of Number Sense using the NWEA MAP assessment. – Poor Goal Sample: I want my students to be better at Math.

Q.2 Why did you select this goal? What data sources did you use to determine that this goal is relevant for your students? Tips – Data sources can be standardized such as: MAP, DIBELS, AIMS, DRA,… – Data sources can be teacher generated: Pre- assessment data (Reading A to Z), Rubric, Continuum, Prior grade level exit test data,… – Make sure data relates to goal statement

Q.3 State your grade/subject. List the grade level(s) you teach and the subject area

Q.4 Describe your assessment and how it aligns with your goal. Tips – Assessment can be teacher-made or standardized – Good Sample: For my assessment, I will be using the NWEA MAP results for the Fall, Winter and Spring testing cycles. I will be using the specific sub-category of Number Sense which aligns with my goal of increasing student achievement with Number Sense. – Poor Goal Sample: I will use MAP scores.

Q.5 List the specific state standards/Common Core Standards and specific performance objectives completely (no abbreviations or numbers only). Tips – No abbreviations or numbers only – List only those that are the focus of this particular goal and assessments

Q.6 Include your measurement tool (s) (e.g., rubrics, continuums, criteria list). If you are using NWEA MAP or DIBELS assessments, please describe how you are utilizing the results and how you are setting the growth targets for your students. Be sure to use the appropriate tools available from the publishers of the assessments to make your decisions. Tips – Sample: I will be using NWEA MAP assessment in the Math sub- category of Number Sense. I will be using the results from the Fall, Winter and Spring MAP testing to assess student progress. Growth targets will be established according to the NWEA growth projection based on expected growth from Fall to Spring.

Q.7 List the specific 21st Century Skill (s) in your instruction and that will be evidenced in resulting student work. Give an example that you plan to include in your instruction. – Sample: A 21 st Century Skill students will use is Critical Thinking. Students will effectively analyze given content for bias and assumptions. Tips – Refer to the 21st Century Skills list
