3. 2- The first screen appears. Select “Contamination survey” 5-Dec-20113

4. 3- The Inventory List menu opens. Select “Add Contaminations Survey”. 5-Dec-20114

5. 4- A form appears. Select the triangle mark to see the calendar 5-Dec-20115

6. 5- Enter the date of the survey 5-Dec-20116

7. 6- Click in the blue “i-link” field 5-Dec-20117

8. 7- The List of Labs is loaded. Select the lab where you are working 5-Dec-20118

9. 8- The “Lab” field is automatically updated. Enter the name of the person who made the survey 5-Dec-20119

10. 9- Open the list of detector of your lab. 5-Dec-201110

11. 10- Select the detector used for the contamination survey 5-Dec-201111

12. 11- Click the mark to select “yes” or “no” 5-Dec-201112

13. 12- If a contamination was found, inform what was done. 5-Dec-201113

14. 13- Save your information 5-Dec-201114

15. 14- The last contamination survey appears at the top of the list. 5-Dec-201115

16. 15- Click the “BACK” button or log-off. 5-Dec-201116
