Free Choice of Provider CCBDD Board Policy: –Maximize use of federal Medicaid dollars via enrollment of individuals on Medicaid Home and Community-Based Services (HCBS) waivers –Individuals receiving or in need of services shall apply for a waiver at the time a slot becomes available. Adult Day Services and Supported Employment are some of the many services available under the Level 1 and Individual Options waivers. Free Choice of Provider: –Outlined in Ohio Revised Code 5123:2-9-11) –Outlined in Ohio Revised Code (5123:2-9-11) –Waiver recipients have the ability to choose any qualified and willing provider of waiver services Service and Support Administration Rule (ORC 5123:2-1- 11): –the SA is to “ensure that individuals are given the opportunity to select service providers from all willing and able providers in accordance with applicable state and federal laws”. –The SA is not able to choose a provider for an individual. They can assist the individual/family with interviewing providers the individual has selected.

Free Choice of Provider Ohio Department of Developmental Disabilities maintains a list of certified waiver providers, which is available on their website (www.dodd.oh.gov). www.dodd.oh.gov –OFFICIAL list of all certified providers throughout Ohio Support Administrators share information with families about the DoDD list. In addition, in Cuyahoga County, they share information about the Provider Search Tool (PST) (http://providers.cuyahogabdd.org) which provides further information on providers in this county. http://providers.cuyahogabdd.org –Providers on the Cuyahoga PST have are certified and interested in providing services in our county. They maintain information about their agency on our website.

Additional Features Provider Information page – gives potential providers links to DoDD information on how to become a certified waiver provider. FAQ page – provides answers to frequent questions from families. Links to provider’s websites and main office location CCBDD staff can “manage” referrals – we review for content before approval. Privacy features include secure provider login, and removal of identifying information like address or last name. Providers can designate multiple staff to view referrals (one person may review referrals for HPC/residential supports, another may view referrals for Adult Day Support). This allows faster response from providers. Providers & Support Administrators can refer technical questions to a special email address for quick response The tool allows REAL time posting of referrals and responses from providers = faster service and supports to consumers!
