There may be instances where changes to Prevention & Detection are insufficient to exact the necessary Improvement Manufacturing Process Improvements In these instances a Fundamental Change to the Process as opposed to a Process Improvement may be required Fundamental Changes to the Process or Product are outside the Scope of Process Improvement. PSI does however provide the facility to identify Potential Process Changes. Fundamental Changes to the Process may effect the 1 st Why Functions the 2 nd Why Requirements or the 3 rd Why Failure Mode
