Staff: o One full-time NP o Three part-time NPs o One registered nurse o Two front office staff o One collaborating family practice physician (not on site, always available by phone for any questions or concerns) Who Are We?

o Minor Injury or Illness o Comprehensive Physicals (School and Sports Physicals included) o Well-Woman Exams o Immunizations, Drug Screens, TB Skin Testing o Chronic Illness Management o Laboratory Services (including titers) o Health Education and Promotion o Referrals Services Provided

o Required by Illinois Law for college entrance: Td/Tdap in the last 10 years MMR (two doses) Required: Students must turn in your Certificate of Immunity forms to the Health Center no later than August 26 th o Optional, Highly Recommended for young adults: Meningitis: one vaccine Hepatitis B: series of 3 vaccines Varicella (Chicken pox): series of 2 vaccines Hepatitis A: series of 2 vaccines HPV (Gardasil): series of 3 vaccines For more information on these vaccines and the diseases they are preventing, please visit http://www.cdc.gov/vaccines/ http://www.cdc.gov/vaccines/ SXU Student Immunizations

o PLEASE bring all prescription medications with you, including inhalers and nebulizers o We recommend that students have a “health kit” with them at school which includes things like common over-the-counter medications (especially for cold/allergy symptoms) and remedies. o Keep name/number of primary care provider/specialists in wallet o Optional: Fill out voluntary disclosure of medical condition form (you can pick one up at the Health Center) Reminders for Students
