2 About the Supplemental Questionnaire What is the Supplemental Questionnaire? The Supplemental Questionnaire is simply a questionnaire asking certain applicants for additional information to enhance their application. UC Berkeley Admissions does this via an email, sent mid-January. Who receives the Supplemental Questionnaire? Only about 3,000 of the 50,000+ applicants will receive an email asking them to complete a Supplemental Questionnaire. There are 3 different kinds of questionnaires; which one an applicant receives depends entirely on what information students provide us in their application. 3 About the Supplemental Questionnaire Does the Supplemental Questionnaire mean an applicant is accepted or denied admission? The Supplemental Questionnaire does not reflect whether or not an applicant is accepted or denied. There are many rumors about this being either a “Golden Ticket” or a “negative sign.” These rumors are false. Should an applicant fill out the Supplemental Questionnaire? Providing this information can only strengthen an applicant’s case. This is a unique opportunity for an applicant to tell us more about their personal path to college without negatively affecting their application. 4 Filling out the Supplemental Questionnaire An applicant can access and fill out the questionnaire by logging onto myBerkeleyApplication with the link provided in the email they received. myBerkeleyApplication also may be accessed via our website: http://admissions.berkeley.eduhttp://admissions.berkeley.edu Applicants should provide as much information as they are able to. If we sent a student this questionnaire, it means we would like him or her to provide additional information about individual circumstances. All information will be kept confidential. Applicants should provide fall semester grades if they have them. A letter of recommendation is optional. A recommender can fill out an online form at the link provided in the email. Students should choose a recommender familiar with their academic work, special talents and/or personal circumstances. Office of Undergraduate Admissions | 1
