1. Medication Information What you need to know about having medicine at school

Prescription Medications O Must be in original container with current label O Label must have students name O Correct dosage and administration times must be listed and correspond to the time you want it given at school. O Parent/guardian permission card filled out O MAY ONLY BE BROUGHT TO SCHOOL BY AN ADULT.

3. Daily Medication O May be given to any student requiring daily prescription medication O Must be a current prescription, labeled with student name, dosage and specific time needed O Permission to administer card filled out by parent/guardian O Medication brought to school by parent/guardian

4. Over the Counter (OTC) Medicine ( Tylenol, Ibuprofen, Cold Medicine etc. ) O Must be in original container with label (no baggies of medicine please) O All medicine (except cough drops) MUST be kept in nurse’s office. May be brought to school by a student as long as permission card filled out by parent/guardian O Cough drops may be brought and kept with student in locker, no permission card required.

5. Emergency Medication O All Emergency medication will be kept in the nurses office unless a signed note from physician stating otherwise (carrying an inhaler) O All Emergency medication will be sent on field trips (includes Outdoor Education) O Medication must be brought in by parent/guardian with permission card filled out. O Medication must not be expired!

6. Please refer to your student handbook for the complete rules and regulations regarding medications in the middle school. All medicine will be required to go home at end of school year, we do not keep over the summer. Parent needs to pick up prescription meds, OTC medication will be sent with student
