23 Top 5 DIA Coding Issues in Ohio 1 Resident has a diagnosis of Asthma, COPD or Chronic Lung Disease (I6200) but has no shortness of breath or trouble breathing with exertion (J1100A) or when sitting (J1100B) or when lying flat (J1100C). 2 Resident is at risk for developing a pressure ulcer (M0150=1) and has a pressure-reducing device on the bed (M1200B) but not on the chair (M1200A). 3 Resident receiving antipsychotic therapy (N0400A) with no diagnosis of schizophrenia (I6000) or Manic depression (bipolar disease) (I5900) or psychotic disorder (I5950) or Post Traumatic Stress Disorder (I6100), and no hallucinations (E0100A) or delusions 4 Resident has weight loss and is not on Hospice (O0100K not checked) yet is not on a physician prescribed weight loss regimen (K0300=2). 5 Resident transfers with hands on assistance of 1-2 persons (G0110B2=2, 3), yet balance assessment for transfers indicates no need for assistance (G0300A=0, 1 and G0300E=0, 1).
