February 2010 9 APRIL 2010 Exclusions Business Ventures: “Activity which produces or designed to produce revenue” Preparing or filing income tax returns Worker’s Compensation Law or Unemployment matters Administrative hearings, consultations, and civil and criminal legal actions, any part of which: (a) involve the State of Georgia, either as plaintiff or defendant, or any of its agencies or departments, or any of its insurers, or (b) relate to your employment Services to a spouse or dependent against the plan Participant Charges for services involving defense of child support recovery or defense of driving under the influence of alcohol or drugs Fines, title insurance costs, court costs, court appointed attorneys, filing fees, subpoenas, assessments, penalties, expert witness fees, and other related expenses (e.g., facsimile, copy expense, postage, long distance phone charges, mileage, etc.) For the complete list, please refer to the Summary Plan Document
