Exercise 1, 2 and 3 offers you an opportunity to demonstrate how well you understand the topic and provides you with dedicated time to illustrate your new knowledge in an information flow diagram that can be included in your learning plan

Identify how your organization performs patient demographic matching for document sharing. Systems need to talk to each other for many reasons and health care is increasingly dependent on the integration across health systems and using standards to ensure effective and accurate data exchange Exercise 1 What situations require you to find and access documents at other locations. What kind of issues do you need to consider in the process of this search and retrieval? What standards can you rely on to accurately search for and find the right set of documents? Exercise 2 Consider the types of clinical documents that need to be transferred between your practice area to a clinician in another location. What issues do you need to consider as the sender? What standards can you rely on to ensure that the documents arrive safely at the intended destination? Draw a simple information flow diagram.
