1. Online Open Enrollment with HRC Total Solutions Online Enrollment in your Health Savings Account (HSA)

2. Online Enrollment Your online open enrollment period is November 24, 2014 to December 12, 2014

3. Online enrollment is a step-by-step process comprised of eight steps:  Profile  Dependents (optional)  Eligibility  Coverage Level (Individual/Family)  Payments  Beneficiaries  Terms and Conditions  Summary

4. To begin enrollment: Go to the Secure Consumer Portal provided by HRC Total Solutions: https://employee.hrcts.comhttps://employee.hrcts.com The portal is secured using industry standard 128 bit SSL encryption which is identified by the HTTPS:// in the address bar of the browser window. Enter your User ID and Password Your User ID is your CC5 email address and your Password is sum1 (password is case sensitive) Note: The system will prompt you to enter a new password and security question.

5. Click the enroll now link

6. Click ‘Next’ to continue. Review the Summary of Pre-Tax Benefits.

7. Please select the HSA Plan Year for enrolling in the HSA

8. Click ‘Next’ to continue. Read and Agree to all terms and conditions.

9. Click ‘Next’ to continue. Your social security number is required to set up your HSA. Complete the required fields marked with a red asterisk.

10. If applicable, enter dependent information and click ‘Add Dependent’. Click ‘Next’ to continue.

11. Review the HSA Qualifications and certify that you are eligible for an HSA.

12. Click ‘Next’ to continue. Choose your coverage level (Individual/Family)

13. Click ‘Next’ to continue. Select your Payment Methods.

14. Designate Beneficiaries and the Share Percentages. Click ‘Add Beneficiary’ to add each beneficiary. Click ‘Next’ to continue.

15. Review your information carefully. Click ‘Next’ to continue.

16. Complete the HSA Account Creation Authorization. Click ‘Submit Enrollment’.

17. Important Note Be sure to complete the entire process until you see the Confirmation page or your information will be lost. Please note if you are going to enroll in the limited FSA or DCA, you need to start back at the beginning and follow the FSA PowerPoint instructions You can always return to make changes later !
