Ideally…Wouldn’t it be great if Community organizations and health care providers were working together to improve end-of-life care (EOLC) People were aware of EOLC issues and knew where to find resources before a crisis Hospices, palliative care organizations, coalitions and others had resources to conduct community outreach initiatives
