2 What is an ADRN? ADRN-Aging Disability Resource Network: The larger community of partners including Area Agencies on Aging ADRN’s are a collaborative effort from the national to the local level that serves as “no wrong door” systems ADRN’s are not limited by state programs, but include all programs The goal of the ADRN is to keep individuals in the community

18 Ohio Benefits Long Term Care ◦ Person-Centered:  SEPs will work together to ensure extended coverage hours for North Central Ohio ADRN  Availability  Monday-Thursday 7a.m. - 7p.m.  Friday 7a.m. - 5p.m.  Saturday 8a.m. – 5p.m.  Accessible by consumers  Willing/Able to complete LTCQ on every consumer/call that comes through their door.  SEPs will provide a similar experience regardless of which agency the consumer initiated contact with

19 Ohio Benefits Long Term Care Access & performance ◦ Once the system is accessed a Long Term Care Questionnaire (LTCQ) will be completed with the consumer. ◦ LTCQ  Housed within LOTISS—Linking Ohioans to Independence Services & Supports  A web based system developed by Harmony for Ohio  Incorporates use of Trigger Questions & Algorithms'  Identifies potential for Medicaid eligibility  Determines:  the need for a level II Assessment for waiver services:  or determines list of appropriate referrals to be made  MH  VA  Traumatic Brain Injury  Brain Injury Assoc.

21 LTCQ Summary: ◦ 7 page questionnaire on line ◦ Developed to screen for multiple Long term service & support needs ◦ Determines if Assessment is needed ◦ Determines potential for Medicaid eligibility.

22 Ohio Benefits Long Term Care ◦ Once LTCQ completed=consumer gets linked with a support navigator (if needed) within the SEP ◦ Support Navigators provide options counseling to consumers according to the outcome of their LTCQ.

23 Ohio Benefits Long Term Care Support Navigation  Assists consumer with referral for level II Assessment (ACAT/CCAT)  Assists consumer with referrals to community based LTC services & supports (ADRN Partner Agencies)  Assists consumer with pending Medicaid application & eligibility process

24 Ohio Benefits Long Term Care ◦ Intended outcome of new LTCQ & Support Navigation:  Consumers no longer get lost in the maze of our current system!

25 Ohio Benefits Long Term Care Quality Measurement & Continual Improvement: ◦ SEPs will be expected to meet performance measurements established & monitored by the State.  Expectations for the completion of the LTCQ & support navigation  Example: Return phone calls by close of next business day  Monitored within LOTISS

27 Ohio Benefits Long Term Care Take aways for ADRN partners: ◦ New & improved consumer system that will begin: 7/1/15 ◦ Consumers in need of Long Term Services & Supports need to go through:  One of the region’s 6 SEPs  State wide 800#  State wide website ◦ Consumer’s will have access to Support Navigation  Options counseling—with referrals to other agencies  Linkage to level II assessment  Assistance with Medicaid Eligibility process

29 Strategy Cafe Directions: 1. There are 5 questions located at 2 separate tables each, through out the room. 2. Participants are asked to brainstorm ideas surrounding the question at their table with the other participants at the table. 3. Each table has a designated Scribe who will record the ideas of the group 4. When time is up please relocate to a table with a question you have not yet had a chance to brain storm 5. Once we have rotated 5 times we will take a break & tally common themes for report out.
