13 Linkage with DSPS  Embedded within the Disabled Students Program Services  Attend DSPS staff meetings, conduct joint student orientations, events, and activities  Share DSPS data base for student information  Will have shared program space in 2011on the campus The Transition To College program is one of the accommodations offered by the Disabled Students Programs and Services at The College of San Mateo

14 Review of Program Adherence to Fidelity Measures (256 out of 270)*  Materials are displayed throughout locations.  New staff are orientated to Supported Ed. Mission.  Ongoing attendance in program staffing.  Educational assessments and educational plans are developed for each student.  One-One assistance is provided for enrollment, financial aid and other student resources.  Outcomes are tracked and shared on a quarterly basis. * Supported Education Fidelity Scale~The University of Kansas

15 Outcomes For the period of 2007-2009  Total number of students served- 129  Total number of service hours provided- 431.5  A total of 30 classes: ‘Transition to College’, ‘Skill Development for Career Growth, ‘Peer Counseling’ and ‘Advanced Peer Counseling’ are offered  Engagement activities such as regular social activities and open houses are conducted year-round.  Program staff outreach to mental health programs in San Mateo County  Contracted to provide Supported Education Services to Transition Age Youth Full Service Partnership
