5. Cruise Coupons: Note: Cruise discount coupons are to be submitted prior to travel. Credit for the amount of the appropriate discount will be issued to the purchasing credit card account or a check mailed to the purchasing customer or a check mailed to the purchasing customer. 6. Domestic Airfare Rebate Coupons: Note: Air fare rebate coupons are to be submitted upon completion of travel. Boarding passes must be included. Credit for the amount of the appropriate rebate will be issued to the purchasing credit card account or a check mailed to the purchasing customer. Each book contains 8 rebate coupons for domestic air fare purchases. Each coupon is for one ticket. An individual traveling alone would use one rebate coupon. If a family of four is traveling, four rebate coupons would be used as four tickets are purchased. Applies to round trip purchases only. Tickets must be booked through Bill Brown's Plaza Travel. Each coupon has the following offer: Purchase a ticket with a minimum cost of $200, receive a rebate of $25. Purchase a ticket with a minimum cost of $300, receive a rebate of $50. Purchase a ticket with a minimum cost of $400, receive a rebate of $75. Purchase a ticket with a minimum cost of $500, receive a rebate of $100. 8. What has to happen for the customer to get the travel discount coupon book? 1.Customer makes purchase. Store provides Customer a flyer explaining how they will receive their TRAVEL DISCOUNT COUPON BOOK. (see postcard, right). Stores will be sent flyers to be given customers at time of sale. If, for any reason, flyers are not available, it is possible to go to the Current News page at www.fmgbuyinggroup.com/currentnews.aspx to download and print a copy of the flyer which can be given to the customer. 9. What has to happen for the customer to get the gift card? 2. Record of qualifying sale. At the conclusion of the event, each participating retailer must provide to FMG a listing of all Customers that have made a qualifying purchase during the event. This simple spreadsheet requires only a modest amount of information-- name, address, phone, retailer, branch, date and items purchased. The last date to submit a coupon book request is July 31, 2015. The sale must be completed and FMG must be notified prior to that date.

10. What has to happen for the customer to get the coupon book? 3. Purchased items are delivered to the Customer. Store notifies FMG using the Dealer Sea and Air spreadsheet (see prior page) that the Customer should be sent a COUPON BOOK. FMG notifies Capitol Marketing that the coupon book is to be released to the Customer. Capitol Marketing mails the coupon book to the customer using the address information provided by the retailer. Note: Stores will not receive travel discount coupon books. Coupon books will be sent directly to consumers. There is no charge to the consumer or to FMG member retailers for the coupon books. Stores are only required to provide the list of qualifying customers. How To Use Your Airfare Rebates 1. When making your travel requests, please have dates and preferred times ready to facilitate service. Airline ticket prices change without notice. 2. All tickets must be purchased via credit card at time of booking. Availability and fare quotes are not guaranteed unless paid in full and ticketed. All bookings not paid in full will be canceled. 3. To receive your rebate you must submit your Air Fare Rebate Coupon along with your boarding pass and itinerary within 30 days after travel has been completed to the address below. 11. Cruise Rebate Terms and Conditions Each check is valid only towards one cabin per one booking on a cruise. Applies to cruise fare only, per guest, based on double occupancy for new cruise bookings through Capitol Cruises. Government fees and/or taxes are additional. 15. More Details Customers will receive a travel discount coupon book from Capitol Marketing/Meridian upon completion of the sale and after notification from the retailer. Offer valid for purchases made between March 19, 2015 and March 31, 2015 only. Prior sales excluded. Upon receipt of the completed spreadsheet from dealer, Capitol Marketing/Meridian will process the request and mail the coupon book directly to the consumer in 2 to 4 weeks. Once issued a travel coupon book cannot be withdrawn. In the case of a return or cancellation where a coupon book has been issued, the value of the coupon book issued should be deducted from any refund by the retailer. Travel checks contained in the coupon book are completely transferable, but may not be resold under any circumstances.
