Intuitive recognition that housing, social care and health are interlinked as pillars for H&WB H&WBB/ICB driving change & a focus on priorities and outcomes HSP brings people together with an interest in housing services – but limited health input so far ITF agenda makes it even more important to understand where housing can make a difference HSP commissioned Chartered Institute of Housing (CIH) in Spring 2013 to identify a local housing offer to health Cross agency group steering this work HSP has considered report and suggested 5 areas for joint work to make a difference

Where Can We Start?  Involve housing options in the hospital discharge pathway so that housing barriers/solutions are actioned quickly  Establish a handy persons support service – linked to previous item but has wider benefits around prevention  Take the First contact Scheme into GP surgeries to signpost people to advice and services  Secure money to deliver a targeted affordable warmth programme  Develop model for joined up holistic services for older people's support and care
