3 FUNCTIONAL INDEPENDENCE MEASURE SCALE Independent: 7= Complete independence: activity performed safely, w/o modification, assistive devices and within reasonable time 6= modified independence: Requiring the use of a device but no physical help -no help is needed but the person takes more than a reasonable amount of time to do a task or may complete the task in an unsafe manner.

4 FUNCTIONAL INDEPENDENCE MEASURE SCALE Dependent: 5 = Supervision or Setup: no physical assistance needed. Requiring only standby assistance or verbal prompting or help with set-up =the person only needs someone to standby and cue or coax him/her (without physical contact) so that he/she can do a task. A score of "5" can also be obtained if a helper is needed to set up items or assistive devices for the person 4 = Minimal contact assistance: Requiring incidental hands-on help only (subject performs > 75% of the task)

5 FUNCTIONAL INDEPENDENCE MEASURE SCALE 3 = Moderate assistance: subject requires more than helping/ touching & expends 50-75% of the effort required 2= Maximal Assistance: subject requires 25-50% of the effort 1 = total assistance: Subject expends 0-25% of the effort or is unable to do the task

6 Patient’s Classification 6= modified independence: Requiring the use of a device but no physical help -no help is needed but the person takes more than a reasonable amount of time to do a task or may complete the task in an unsafe manner.
