Progress Update July 2015 Signed off Clinical Memorandum of Understanding Work progressing on development of Out of Hospital model – Pilot sites to be implemented from October 2015 Integrated Quality and Outcomes Framework further refined Further detail developing around enabler workstreams (Estates, IT, Workforce and OD) Programme resource strengthened: – Programme Director appointed – Workforce, OD and Marketing Communications and Engagement resource secured Progress on implementation of a number of schemes

Progress on Out of Hospital Model Pilots Workshops in both NL/NEL GP’s, NLaG Community and Acute, mental health providers, Local Authority, other providers involved in discussions Consideration of: – Locality Geography, GP practice size etc – Intelligence required to inform planning – Integrated services that build on what is currently in place – Processes between generalist and specialists that aid integrated working – Barriers to progression – Estates, payment mechanisms etc

Enabling Workstreams Community estates mapping underway Workforce planning lead appointed Marketing, Communications and Engagement lead appointed and external marketing support secured HLHF Transport session scheduled in August to consider Patient Transport Service specifications. Transport workstream lead to be identified to co- ordinate HLHF transport strategy to align with workplans. HLHF Workshop session scheduled for 3rd September for programme board, clinical leaders and enabling workstream leads to further build plans for out of hospital pilot sites and reflect on acute hospital thinking

Implementation update (recently live and upcoming) Extra Care Housing (NEL), 1 st Phase - Live 7/7 Community equipment service - Live Home from Home Service(NEL) – Live Frail and Elderly Assessment Unit (NL) to be implemented from September Integrated ‘Out of Hospital Pilots’ from October Diabetes “Super 6” model (NL) to be implemented from November

Next Steps Local teams to further develop out of hospital pilot models Sign off of HLHF Key Performance Indicators and incorporation into programme reporting Key programme messages refined and refreshed for public engagement purposes HLHF Programme workshop on 3 rd September NLAG Board workshop 29 September
