Qualifications Currently uninsured Live in the communities of Magna, West Valley City or Kearns Household incomes of less than $35,000. Have a current medical need.

Utah Partners For Health Covers The Following Medical Needs Family Practice Medical care –Acute care – illness, infection, fractures, lacerations, etc. –Chronic illness care – Diabetes, Hypertension, etc. –Mental illness care – Bipolar, ADHD, Depression, etc. Counseling Services Dental Care –Urgent dental needs for primary students

Eye Care –Medical referrals to an eye doctor –Vision care for students Physical Therapy Lab Services Ancillary Services –Ultra Sound –Echocardiogram –Pharmaceutical Assistance –Referral Service –DXA Scan

Prenatal Clinic 11 Prenatal Visits 2 Postpartum Visits Labwork –HIV Panel –Strep Gp BCult/DNA Probe –Chlamydia/GC Amplification –Pap IG/rfx HPV –Prenatal Panel (CBC w/ diff/platelet, Complete Metabolic Panel, Lipid Panel, TSH, HCG Beta, Subunit, Qual Serum, Urine Culture Routine. Ultra Sound

Mobile Medical Clinic Model –Partner with Community Based Organizations. –Reach Disenfranchised Groups. –Educate the Patient and The Community Based Organization’s Leadership.

Mobile Medical Clinic 31 ft trailer with two patient rooms. Well-checks to all ages with emphasis on diagnosing and treating diabetes, hypertension, thyroidism, lipidemia, and asthma. Focus on developmental evaluation for children. Breast, colon, and prostate cancer screening for adults. Minor acute care. Mental illness care dealing with depression, anxiety, ADD, and bi-polar disease. Smoking cessation services and education on the dangers of secondhand smoke.

Early detection of pregnancy and pre-natal care Early detection of coronary artery disease risk. Coordination with state agencies to provide culturally sensitive patient education. Educating the CBO or agency on the health care safety net and prescription assistance programs. Educate patients about follow-up care to Utah Safety Net providers including Utah Partners for Health.
