3 LSST Camera Safety Goals
Purpose Prevention of unwanted events Integrate safety into LSST Camera Project from cradle to grave Objective No recordable injuries (TRCs and DARTs) No environmentally significant releases Protection of equipment “Prevention of unwanted events” sounds like a difficult task, but our safety approach has worked very well with other projects. No events (injuries, releases, loss of equipment) a half-dozen projects. LSST Safety Council Camera Review • Bremerton, WA • 2015

4 ES&H Requirements Documents
DOE O 413.3B, DOE G C LPM-18 LSST Project Safety Policy Provides top-level objectives, requirements, and safety program organization, management and responsibilities Establishes LSST Safety Manager and LSST Safety Council LCA-138 LSST Camera Performance and Safety Assurance Plan DOE O = “Capital Assets” order. 5 ES&H Requirements Documents
LCA-138 (LSST Camera Performance and Safety Assurance Plan)… “Includes the Camera system safety program, environment, safety, and health (ES&H) plans and the means by which personnel and environmental safety processes are managed across the camera collaboration, and the quality management system for the project.” Details the process used to manage the Performance and Safety Assurance Programs for the Camera Project LCA-31 LSST Camera System Safety Program Plan LCA-14 LSST Camera Hazard Analysis Report LCA-15 LSST Camera Hazard List LCA-(various) LSST Camera Hazard Reports LCA LSST Camera Operating and Support Hazard Analysis Institutional Safety Implementation Plans DOE O = “Capital Assets” order. Institutional Safety Many U.S. institutions; one in France Project is managed as a single unit All subsystem managers report to the project manager follow the plans, standards, and processes laid out thereby Key institutional personnel support subsystem managers and leads by implement local environment, safety and health regulations and policies DOE O = “Capital Assets” order. DOE G = ISMS Guideline LPM-18 = Camera-specific is the driving observatory document LCA-138 is a Camera-specific safety management roles-and-responsibilities document LSST Safety Council Camera Review • Bremerton, WA • 2015

8 Safety Roles and Responsibilities
Camera Project Manager Overall project authority: project planning and scope; performance, and quality objectives on schedule within the approved budget; ES&H procedure compliance, and for coordinating project activities; staffing plans and actions. Safety Officer Reports to CPM; influences design for human and hardware safety minimizing the overall hazard level of the camera design and operations Systems Integration Manager Provides technical leadership; directs subsystem development teams through requirement development and tracking; assures subsystems are compatible; oversees, from the Camera side, all aspects of camera-telescope and camera-data management interfaces, as well as flow-down of LSST system-level requirements. Subsystems Managers Institutional Safety Officers DOE O = “Capital Assets” order. DOE G = ISMS Guideline LPM-18 = Camera-specific is the driving observatory document LCA-138 is a Camera-specific safety management roles-and-responsibilities document LSST Safety Council Camera Review • Bremerton, WA • 2015

9 Safety Roles and Responsibilities
Subsystems Managers Camera Subsystem Managers or their designee are responsible for integrating the safety and quality assurance programs into their subsystem; (supported by the QA Manager, Camera Safety Officer and Systems Integration Manager, but are ultimately responsible for their subsystem Institutional Safety Officers Support institutional line management to ensure that the institutional plans and processes are implemented on Camera work at that institution. The Institutional Safety Officer may report directly to the line management responsible for Camera work at the institution or to higher-level ES&H officials, depending on the organization’s management structure. They are also responsible to work with the Camera SE to ensure that local processes meet camera requirements, as well as with institutional subject matter experts, building and area managers, and other personnel that support or oversee Camera work at the institution. Staffing Institutional Safety Officers/Managers Institution Safety Manager/Point of Contact LSST Project Chuck Gessner LSST Camera Joe Kenny Observatory (Chile) Sandra Romero BNL Matthew Rumore IN2P3 Pierre Karst Harvard Peter Doherty UPENN Rick Van Berg We have a number of excellent safety resources at SLAC and at our sister institutions. Gessner is 100% I work with Frank, whom I’ve succedded as safety manager. Ms. Romero is ½ time, we’re sharing her with the 8-m Gemini telescope next door. LSST Safety Council Camera Review • Bremerton, WA • 2015

11 System Safety Engineering
SYSTEM SAFETY - The application of engineering and management principles, criteria, and techniques to achieve mishap risk as low as reasonably practicable (to an acceptable level) within the constraints of operational effectiveness, suitability, time, and cost MISHAP RISK- A measure of the expected loss from a given hazard or group of hazards. Risk is the combined expression of loss severity and probability LSST Safety Council Camera Review • Bremerton, WA • 2015

12 Safety During Integration and Testing
Focus of LSST Camera Operating and Support Hazard Analysis (LCA-11661) Engineering controls built into systems and work areas Direct application of SLAC work planning and control requirements: All work is planned (using SOPs, ATAs, JSAs, WIPs, and/or eTravelers) All work is authorized (by worker’s functional supervisor or lead collaborator All work is released (by SLAC Area Manager) Administrative controls Training Access control Signage Daily, direct oversight by Camera safety personnel LSST Safety Council Camera Review • Bremerton, WA • 2015

13 LSST Safety Council Camera Review • Bremerton, WA • 2015
Review Process Camera Safety Officer formally assessed subsystems at Camera design reviews reviews, viz.: Cryostat FDR, Corner- and Science-Raft Pre-CD-2, Auxiliary Electronics Pre-CD-2, DAQ and CCR FDR, Filter Exchange System FDR, Filter FDR, I&T PDR. Subject matter experts are engaged for assessment of mitigations for special hazards. Oxygen deficiency Hoisting and rigging Pressure safety Independent entities review Camera safety at various levels of detail during CD-2 and CD-3 (Directors’ and DOE Reviews) Camera FDR LSST Safety Council ES&H expertise will be similarly enlisted during upcoming reviews LSST Safety Council Camera Review • Bremerton, WA • 2015

14 LSST Safety Council Camera Review • Bremerton, WA • 2015
A safety analysis has been completed on the L3 Assembly and approved by LLNL, SLAC and the LSST Project Office LSST Safety Council Camera Review • Bremerton, WA • 2015

15 LSST Safety Council Camera Review • Bremerton, WA • 2015
Summary Documentation for the ways and means of implementing safety across the project are in place and in effect Dedicated personnel are employed across the project to assure adherence to those documents Integration and testing will progress safely The use of the Camera Safety Officer and independent reviewers in the Camera design and planning is extensive and adds greatly to ES&H The Camera team is duly prepared to safely begin assembly LSST Safety Council Camera Review • Bremerton, WA • 2015
