Clinical Health Services Services provided through the Health Fee http://www.snc.edu/health/clinicalservices/ RN assessments and tests for strep, mono, influenza, iron, glucose, etc. General health promotion practices and strategies Counseling services Fees for Service are billed to your insurance Medical services provided by Physician Assistant, Nurse practitioner, or psychiatrist Immunizations, prescriptions, procedures, durable medical goods Transportation to health appointments Metro Bus (free) Cab Vouchers (cost billed to student acct.)

Required immunizations (registration hold will occur without these) Measles, Mumps and Rubella, MMR (2) Recommended immunizations Tetanus/diphtheria,/pertussis, Tdap (1) Hepatitis B (3) Meningitis (Menveo, Bextero); if initial meningitis vaccine was given before age 16, get a booster Polio childhood series Enter all other immunizations received, especially related for travel outside of US

Requirements (continued) 3.Health Insurance Students will be automatically enrolled in the high deductible plan of the WPS/WAICU student health insurance if no insurance policy is input online (your insurance information is the waiver out of the SNC plan).
