2 What Is Lockout/Tagout?  "Lockout/Tagout (LOTO)" refers to specific practices and procedures to safeguard employees from the unexpected energization or startup of machinery and equipment, or the release of hazardous energy during service or maintenance activities.  Before performing service or maintenance and that the authorized employee(s) either lock or tag the energy-isolating device(s) to prevent the release of hazardous energy

3 Types of Lockout Devices Locks Tags Wedges Chains Multi-lock hasps (5 locks per hasp) Valve lockouts Breaker/Fuse lockouts Plug lockouts

5 Energy Sources Electrical Mechanical Hydraulic Pneumatic Chemical Thermal Gravity Other types of energy

6 When Must LOTO Be Used? When servicing or maintaining equipment where: Unexpected energization or start-up could occur Or released stored energy could cause injury to employees

7 LOTO Exceptions Work where hazardous energy does not exist Activities performed during routine production processes Work on cord or plug connected electrical equipment where it is controlled by the employee

8 Typical LOTO Procedures Performing a shutdown, include testing machinery Isolating equipment Applying and removing lockout devices Safely releasing stored energy

9 Affected Employees Employees who Operate Work around Occasionally adjust equipment that is subject to LOTO

10 Authorized Employee Recognizes hazardous energy Types and magnitude of energy in the workplace Means to isolate those types of energy

11 Authorized Employee Responsibilities Repair or service equipment as needed Ensure that all energy sources are locked out Test equipment to verify residual energy is dissipated Place a “Danger—Do Not Operate” tag on equipment

12 Affected Employee Responsibilities Notify maintenance, etc., when equipment needs repair or adjustment Leave all LOTO devices in place Verify equipment is safe to operate following LOTO Follow all safety rules while operating the equipment

13 Authorized Employee Responsibilities (cont.) Obtain assistance when necessary Remove locks and/or tags following LOTO Coordinate multi-shift repair

14 VIOLATIONS OF LOTO Use any lock for lockout Leave keys in lock during lockout Use someone else’s lock during lockout No identification on lock
