Fill in all shaded areas, supply leading zeros if needed, such as: Date: 021012 Each employee is to use the number that Contractor Safety has assigned to them (see Contractor Safety Observation Intervention Card (SOIC) Process). It is suggested to use the last 6 numbers of the ss#. Use leading zeros if employee number is only 4 numbers. (example 005821) SL Code = Company Code (most companies are 99 unless previously specified) Contractors will use “C” Top Section / Front of Card

ONLY fill in the areas that apply to the observation that is being made. For each safe observation that is made one “S” should be filled in. For each unsafe observation that is made one “U” should be filled in. If an intervention is made, one “I” should be filled in. Back of Card

SOIC TIPS How to Fill in SOIC Cards It is your responsibility to assure the cards are filled out correctly. Date & Employee number must be filled in Supply leading zeros if needed, such as: date: 040106 employee number: 002869 Use only black or dark blue ink to fill in bubbles Do not use markers that bleed through the paper Fill the bubbles in completely DO NOT fill the bubbles in like this: Do not use white out on cards Do not staple cards together X

Frequently Asked Questions Who must participate in the SOIC/Observation Process? All on-site Foremen, Superintendents, TOCAS members, Safety Contacts & Safety Managers How many observation are required each month? Foremen/Superintendents – 15 per month Safety Contacts & Managers – 15 per month TOCAS member – 4 per month Who assigns the employee number? Once the Add/Remove employee form is received by Contractor Safety, we will assign a number for the employee. This number will be emailed back to the person that submitted the form Where do we get our Company code? If you do not already have a company code or do not know what your company code is, use “99”.

Frequently Asked Questions Who do we contact to get started? Send the Add/Remove Employee form to ffrcntr@dow.com.ffrcntr@dow.com Where do we get SOIC cards? SOIC cards are available at B-101, EH&S Resource Center, Door G. Monday – Thursday 8:00 AM – 4:00 PM. Where do we turn in the SOIC cards? EH&S Resource Center at B-101. Please pay attention to the red boxes – they are labeled for different items. Place SOICs in the box labeled SOICs. When are SOIC cards to be turned in? Cards must be submitted weekly – each Monday by 3:30 PM for the previous week.

Frequently Asked Questions What happens if the cards are not submitted by the deadline? SOIC cards submitted after the deadline will be scanned but you will not receive credit until the following week. What do you do with the cards after they are scanned? They are all shredded. Why am I not getting credit for my cards I am turning in? There are many reasons: You are using incorrect SOIC # Your company has not set you up in our system (using Add/Remove Employee form) A marker was used that bled through the paper The bubbles were not filled in completely White out was used on the card You stapled your cards together Do we get credit for the rejected cards? No – there is no way for us to know what cards are rejected. Therefore, it is a best practice for you to submit a few more than the required amount and keep an eye on your monthly report numbers.

Frequently Asked Questions How many observations can we have on each card? It is suggested that you complete one card for each observation that you complete. You are only required to do 15 CARDS per month. The number of observations you complete is important but it is not necessary for you to have all blanks completed on the back of the card. Filling out 3 S’s, 3 U’s & 3 I’s for each line is wasting your time. Only complete the observation categories that you actually observe. Who can I contact with questions? The EH&S Delivery Tech that is assigned to your company.
