Step #1 Temporary Employee Checklist and Cover Sheet Fill-out: Department : list the dept. he/she will be working at Employee Name Submitted by: usually the name of the originator check off every document listed once completed Additional Notes: usually used to denote that TEMP. ALREADY HAS DOCUMENTS ON FILE Do not SAVE any documents due to modifications.

Step #2 401 Flat Rate Temporary Employment Form Fill-out: Name Permanent Address, City, State, Zip Working Title and Description of Duties Yes/No to Questions 1-3 Current FTE Anticipated Start Date Appt. End Date Position # Hourly Rate Fund Type Select: State or Foundation Enter Six Digit Fund # and Acct. # Provide estimated salary Route for Signatures Do not SAVE any documents due to modifications. THINGS TO KNOW: 1) ONCE DOCUMENT IS SIGNED YOU WILL NOT BE ABLE TO MODIFY ANY INFORMATION. 2) IF THERE IS ANY MODIFICATIONS NEEDED, BEFORE SIGNING SAVE THE DOCUMENT WITHOUT SIGNATURE; THEN REROUTE. 3) DO NOT SAVE ANY DOCUMENTS FROM HR WEBPAGE DUE TO MODIFICATIONS.

Step #3 401 Temporary Employee Contract Enter all information in blank spaces Make sure the EFFECTIVE DATE matches the date on 401 Do not SAVE any documents due to modifications. Jane Doe

Step #4: For New Temp. Employees The following documents are required to complete the hiring process (see below): I-9 W-4 NC-4 or NC-4EZ NC-4NC-4EZ Direct Deposit Form Criminal Background Check Race & Ethnicity Information Sheet All forms can be found @ Human Resources - Recruitment Human Resources - Recruitment Please do not save any forms due to modifications To open link(s) either view in slideshow mode or right click >> open hyperlink FOR PREVIOUS TEMP.
