Copyright 2013 National Business Coalition on Health “Advanced” State Pilots NBCH support for 3 coalitions (the Minnesota Health Action Group and 2 in TN) to explore implementation of DPP with State (as an employer) and others Both states have: Identified pilot market Gained senior leadership approval Explored vendors to support implementation Engaged team to address challenges Adopted model to conduct DPP “on-site” initially Significant learnings about implementation will be released by Summer 2013
