Items You Need to Submit: Floppy Disk with the ASCII File Air Toxics Certification Statement Discharge Report Cover Letter

FLOPPY DISK Enclose disk with a protective cover Small multiple facilities must be a separate file Make sure ASCII file has been saved on disk Label diskette: Facility Name, TEDI ID, and Reporting Year

CERTIFICATION STATEMENT (Proposed Rule April 20, 2004) LAC 33:III.5107 “Initial and subsequent annual emission reports and revisions to any emission report shall include a certification statement to attest that the information contained in the emission report is true, accurate, and complete and signed by a responsible official as defined in LAC 33:III.502.”

Certification Statement Cont’d Name of facility TEDI ID Number Reporting Year – TEDI or Revision The Certification Statement cannot be included in the body of the cover letter. Separate statement for each facility and reporting year

DISCHARGE REPORTING REQUIREMENTS LAC 33:III.5107.B5. “All discharges to the atmosphere of a TAP from a: safety relief device, a line or vessel rupture, a sudden equipment failure, or a bypass of an emission control device, regardless of quantity, must be reported to the department in the annual emissions report and where otherwise specified in the applicable subchapters.”

Report Shall Include: Identity of the source Date and time of the discharge Approximate total loss during the discharge ……………. LAC 33:III.5107.B5

COVER LETTER Should Contain: ~Company/Facility Name ~Agency Interest Number – AI# ~Facility ID# ~Invoice Mailing Address ~Facility Contact Name and Number

Common Errors to Avoid: Invalid CAS Numbers Line duplications More/Less than 53 characters on a line Invalid Julian date/Reporting Year Missing Air Toxics Certification Statement
