4. MU Objective and Measure Provide patients with an electronic copy of their discharge instructions at the time of discharge, upon request. More than 50% of all patients who are discharged from the hospital’s inpatient or emergency department and who request an electronic copy of their discharge instructions are provided it. MU Measure Type of Measure: Rate The number of patients in the denominator who are provided an electronic copy of discharge instructions. >50% Number of patients discharged from an eligible hospital's or CAH's inpatient or emergency departments (POS 21 or 23) who request an electronic copy of their discharge instructions and procedures during the EHR reporting period. RPMS Reporting Logic Denominator Inclusions: COUNT: each patient HAVING: requested an electronic copy of discharge instructions during the EHR reporting period HAVING: a hospitalization during the EHR reporting period (defined as Service Category of H) WHERE: the Discharge Type was Regular Discharge, Transferred, or Irregular Discharge OR HAVING: an emergency department visit during the EHR reporting period (defined as clinic code of Emergency Department-30 and Service Category of A) Electronic Copy of Discharge Instructions
