SUPPORT PLANNING IN ENFIELD Enfield is committed to transforming the way adult social care is delivered to all user groups – Beacon Council, PCP, DP etc The aim of Support Planning is to identify creative, flexible and user led solutions to meeting assessed need. Support Planning requires a new thinking based on service users maximising their control over how their needs are met using Direct Payments as the preferred delivery system.

SUPPORT PLANNING IN ENFIELD Support planning is a way for people to say what they want to change in their lives The Support Plan is a new document which has replaced the Social Care Plan (not CPA). It has been designed to be completed either by Council staff, staff from partner agencies or by the service user or carer if they wish to complete their own support plan Support planning should focus on achieving outcomes – not prescribed services

SUPPORT PLANNING IN ENFIELD The Support Plan should be used for all people eligible with a personal budget (ie those who are assessed as needing community care services) living in the community. Support plans need to address issues of risk – adult conversation! The focus should be on proportionate support planning. The support plan does not need to be complicated if needs are straightforward. However, it can be enriched through the addition of universal and other low or no cost services that will improve quality of life.

SUPPORT PLANNING IN ENFIELD Support planning is ‘whole life’ planning. It looks at dreams and aspirations Support plans need to be realistic Support planning is led by the user with support and guidance as necessary. If the service user lacks capacity, they should be involved as much as possible with their circle of support What is important to the person is key to good support planning A chance to be creative!
