Reduce “preventable” admissions due to medication
Background In order to implement the Reshaping Care Agenda, the Scottish Government made time limited (3 year) funding available to all local authority areas (the “Change Fund”). Target population – 65 years plus receiving Care at Home recently discharged from hospital.
