2 This evaluation was developed in collaboration with staff from the following departments. NTC Mentors Educational Technology Human Resources Instructional Technology The Education Network Professional Development Annual Evaluation Teacher Observation and Evaluation System (T.O.E.S.)

3 Category 1 Teachers All teachers who were hired after 1/10/10 Probationary or interim teachers are considered Category 1 teachers. The following observations should be completed for Category I teachers in iObservation prior to May 22, 2012. 1 Informal Observation 1 Formal Observation 3 Walkthroughs Please make sure your data is showing up. 4 All teachers who have 3 or more years of service The following observations should be completed for Category 2 teachers in iObservation prior to May 22, 2012. 1 Informal Observation 1 Formal Observation 3 Walkthroughs Please make sure your data is showing up. 10 If the teacher refuses to sign, the administrator places “Teacher Refuses to Sign” in the comment section of that page with the phrase: “Witnessed by: Administrator Name (and one other person usually the confidential secretary or another administrator in that space)”. Once the principal electronically signs, the form will be sent to HR and electronically placed in their personnel file.
