3. Emergency Department Transfer Communications Aim To provide a means of assessing how well key patient information is communicated from an ED to any healthcare facility.

5. RHND Network Self Registration To register for an account, complete the electronic form: https://rhnd-profile.qualityhealthnd.org/register https://rhnd-profile.qualityhealthnd.org/register This gives us the information we need to build your account in the data collection and reporting systems.

6. RHND Network Self Registration After submitting your self-registration, an account will be created for you. You will receive an e-mail with instructions and a temporary password.

12. REDCap Account Validation REDCap is the tool that will be used for data collection. Log in to REDCap using your newly created RHND Account https://rhnd-rc.qualityhealthnd.org

14. The first-time login will send a verification e-mail. After you verify your new REDCap Account, the appropriate data collection projects will be added to your account. This step is a manual process and may take up to 5 business days to complete.

17. Data Submission: Outpatient Encounters Click Outpatient Encounters

18. Data Submission: Outpatient Encounters Click Add / Edit Records

19. Data Submission: Outpatient Encounters Click Add new record This screen also has the ability to search for previously entered records, whether they are complete or not.

20. Data Submission: Outpatient Encounters The first data entry screen contains patient demographics and encounter data.

21. Data Submission: Outpatient Encounters Change the status to Complete Click Save and go to Next Form

22. Data Submission: EDTC Measure Answer each question as appropriate, in accordance with the EDTC Specifications Manual. A link to the manual is available to the left of the data entry form.
