This DBA application features an advance 3 tab form with linked sub forms, multi- dependent dropdown lists along with dependent auto fill fields. It also has the capacity to store and display images or documents.

Once the item is selected, the rest of the related fields are populated.

Clicking the ADD/VIEW CONTACT button will open the Vendor Contact Form to add new contacts.

Note: the Vendor Contact Form will open with related data current to the Main Form

Dependent list are one of the most difficult items to render in an Input Form

Below are 2 sub forms with a dropdown list that will also auto fill the related fields.

v Selecting Add 3 rd Party and Add New TA below will open the associated form for new item input

The Vendor Info tab has all the data that is related to the vendor ’ s status.

Documents and/or Images may be dragged into the DOCUMENT tab as part of the record information.

When the Document field is double clicked, the file will open in it ’ s own given application
