Some stations reporting “errors” not meeting significance criteria in INPO and CDE Data Element Manual, INPO

Validate correct data entered

Definition of a Site Human Performance Event-Free Day (EFD) Reset – When an event, as described in the table below, occurs as a result of either of the following:
an initiating action (error) by an individual or group of individuals (Re) (event resulting from an active error)
an initiating action (not an error) by an individual or group of individuals during an activity conducted as planned (Md) (event resulting from a flawed defense or latent organizational weakness created within the last 18 months)
