IHE Canada Workshop – Sept 18-19 2006 What IHE Delivers 1 Kevin ODonnell Toshiba Medical Systems IHE Structure & Concepts

IHE Canada Workshop 2006 2 IHE Domain Addresses an area of integration issues Radiology, Cardiology, IT Infrastructure, Laboratory Patient Care Devices, Patient Care Coordination Radiation Oncology, Eyecare [ Pick One… ] Brings together relevant professional societies & vendors Each Maintains a Technical Framework

IHE Canada Workshop 2006 3

4 Users IHE Vendors RFP (Profiles & Actors) Standards (DICOM, HL7, etc) Standards (DICOM, HL7, etc) Standards (DICOM, HL7, etc) IHE Integration Statement (Profiles & Actors) C-Thon Results Select IHE Profiles IHE Integration Profiles IHE Integration Profiles IHE Integration Profiles IHE Technical Framework Product with IHE Profile IHE Connectathons IHE Demonstrations Real-World Integration Problems The IHE Process

IHE Canada Workshop 2006 5 IHE Technical Framework The Document for a domain Public: www.ihe.net/Technical_Framework Several Volumes (its large) Vol. 1 – defines the Profiles

IHE Canada Workshop 2006 6 IHE Integration Profile Integration Profile: The Most Important Term Vendors implement them Users buy them / list them in RFPs Each Profile defines a Problem / Solution E.g. IHE Scheduled Workflow Profile (SWF) How do I maintain the integrity of patient demographic & order information and manage workflow in the Radiology Dept? The Profile bundles all the necessary integration for the solution

IHE Canada Workshop 2006 7 IHE Scheduled Workflow Profile Registration Orders Placed Orders Filled Film Folder Image Manager & Archive Film Lightbox report Report Repository Diagnostic Workstation Modality acquisition in-progress acquisition completed images printed Acquisition Modality

IHE Canada Workshop 2006 8 IHE Integration Profile In each Profile in the Tech. Framework: the context is described (Use Cases) (what real-world problem is addressed) the actors are defined (what systems are involved) the transactions are defined (what do they have to do when they talk)

IHE Canada Workshop 2006 9

10 IHE Transactions The guts of IHE - Vol. 2 of the Technical Framework This is the product implementation guide Each Transaction: Addresses a specific exchange of data Selects an existing, open standard (e.g. HL7, DICOM, SMTP, …) Documents how to use the standard to implement the transaction Strictly specified to avoid incompatible implementations (retire options, clarify data model, etc.) Re-usable, may appear in multiple Profiles Defines external interface Does not constrain System internal functions

IHE Canada Workshop 2006 11 IHE Transactions E.g. Query Modality Worklist Transaction [RAD-5] Details Include: Based on DICOM Modality Worklist Management Service Specifies mapping from HL7 Order to Worklist Procedure Step Refines Matching Key and Return Key requirements Defines exchange of data between actors Order Filler Actor Acquisition Modality Actor

IHE Canada Workshop 2006 12 IHE Actor A generalized system function, e.g. : Order Placer Order Filler Audit Repository Implementers choose which Actors included in system E.g. Order Filler In the RIS? In a Broker Box? In the PACS? Sure, just implement the transactions assigned in the Profile Systems may group several Actors e.g. LIS = Order Filler + Lab Automation Manager + Result Tracker Report Creator Patient Identity Source Document Repository

IHE Canada Workshop 2006 13 IHE Options Some Profiles have Options Extensions to the basic functionality Will be listed in the product IHE Integration Statement E.g. PPS Exception Management Option in IHE SWF Order Filler receives details on discontinued Procedure Steps May use that information in business logic, e.g. rescheduling Doctor cancelled procedure Changed procedure for correct charging Patient allergic to media/contrast Patient pregnant Patient refused to continue procedure Patient condition prevented continuing Patient did not arrive Patient died Equipment failure Duplicate order Nursing unit cancel Incorrect worklist entry selected

IHE Canada Workshop 2006 14 IHE National Extensions Technical Framework is Global National Extensions are Appendices in TF: Documents national clarifications and extensions Mostly translations and minor variations Additional codes for national practices E.g. Use of a Provincial Insurance Number Radiology TF - Volume 4 Includes France, Germany, U.S., Italy, U.K. Canadian Extension is near completion

IHE Canada Workshop 2006 15 IHE TF Supplements New Profile Documents Follows Structure of Technical Framework Available on www.ihe.net Published for Public Comment Published for Trial Implementation After becoming Final Text, folded into Technical Framework during annual update

IHE Canada Workshop 2006 16 Users IHE Vendors RFP (Profiles & Actors) Standards (DICOM, HL7, etc) Standards (DICOM, HL7, etc) Standards (DICOM, HL7, etc) IHE Integration Statement (Profiles & Actors) C-Thon Results Select IHE Profiles IHE Integration Profiles IHE Integration Profiles IHE Integration Profiles IHE Technical Framework Product with IHE Profile IHE Connectathons IHE Demonstrations Real-World Integration Problems Its all about the Profiles
