3 3 Use Case The Home Care Nurse (HCN) admits a patient, Ann, newly diagnosed with Diabetes Mellitus (DM) during a hospital admission for a Myocardial Infarction (MI) Ann is has increased anxiety related to her fear of sudden death due to potential of subsequent MI. 5 5 Discussion There has been significant interest national/internationally, article in JAMIA in July on the Canadian model. Nursing colleagues support of an interoperable eHealth/Nursing summary, including international experts.
