NHS England Guidance: By April 2016, we expect that personal health budgets or integrated personal budgets are implemented across health and social care. The descriptor for Personal Health Budgets now includes Integrated Personal Budgets across Health and Social Care. CCGs should engage widely and fully with their local communities and patients, including with their local Healthwatch, and include clear goals on expanding personal health budgets within their published local Joint Health and Wellbeing Strategy. Key Messages and Trafford CCG Responses The focus is not on diagnosis or specific conditions….. The Trafford offer is made to patients with LD, MH, LTC, ABI, Spinal Injury; essentially patients who present with needs that require CCG funding. Investment must be made in Advocacy, Information and Advice, Direct Payment and Managed Account Services. The CCG Business Plan includes investment for these services and plans to work in partnership with local private and third sector organisations. A PHB ‘Pot’ has been established so we can ensure access to and approval of PHBs; and assess the impact on large NHS contracted services Local offers should be informed Working with the Council to develop one local offer Planned change strategy. Co-production Group have the responsibility for the Programme Plan

Staged approach to roll out which does not de- stabilise current systems. Systems in place to track PHB use and inform future commissioning intentions and contracts Health Outcomes Clear Health outcomes and monitoring
