2. There are four types of surveys: Commodity Information, Demand Ordering, Direct Diversion and Hold/ Recall. Commodity Information and Demand Ordering Surveys allow the Commodities Team to gather information regarding needs and future orders of various commodities Direct Diversion Surveys order surveyed commodities and divert these commodities to a processor chosen by the sponsor. 7. Direct Diversion Survey 07/30/2012ODE, Office for Child Nutrition 7 After clicking Continue>>, CATS displays the survey with the Submit Survey button on the bottom of the screen. Use the Submit Survey button to submit the completed survey.

11. Hold/Recall Survey 07/30/2012ODE, Office for Child Nutrition 11 Respond with the amounts of held or recalled commodity items that are in your inventory and verify that you have disposed of them as the USDA Disposition as instructed.

12. Hold/Recall Survey -continued- 07/30/2012ODE, Office for Child Nutrition 12 Read the Special Instructions. If you do not have any of the recalled foods, click the checkbox next to the statement, “We DO NOT have any donated foods identified by the Can/Package Codes below on hand.” If you have any of the recalled foods listed, check the Can/Package code. If the Can/Package code on the items match, enter the number of items in the Cans/Units field.

14. Destroying Held or Recalled Items After you have been notified, watch for an email from the Commodities Team specifying the USDA’s Disposition for the recalled items. 15. Hold/Recall Survey Completion 07/30/2012ODE, Office for Child Nutrition 15 Enter witness names and dates of destruction in the Witness Name and Date Destruction Observed fields. Click the Save button when complete.
