The POPP Organisation The Funding: DH funding £2.4m yrs 1 & 2 (2006 – 2008) DCC/NHS Dorset funding - £1.28m yrs 3 & 4 (2008 – 2010) and £800,000 p.a. (2010 – 2013) The Vision: Build supportive neighbourhoods for older people Seek transitional change to existing ways of working Focus on prevention, health, well-being, and independence The Demographics: Older People aged 50+ living in Dorset excluding Bournemouth and Poole (179,000+)

Strategic Direction and Decision Making Programme Management Community Development and Capacity Building Wayfinders (x54) Signpost and Support – 9 hours per week Talks, events, promotions, practical tasks Leaders (x33) Challenge and change – 7 hours per week Work with older people and others to identify and develop local opportunities based on need Evaluation Capturing the journey Community Commissioning Fund Seed funding to kick start local initiatives and activities

Annual Output Benefits 63,000 contacts with Dorset POPP 23,000 received a service, activity, or information 2000 in-depth information cases undertaken 3000 Activities To date: 600+ new volunteer posts 270 funded projects 1000+ Outcome stories / over 500 stories analysed Improved community capacity Robust partnership working

The Dorset POPP Vision and Desired Outcomes Supportive communities that enable older people to remain living in their own homes for as long as they wish. Desired Outcomes of the Dorset POPP: Older People in Communities… …have housing suitable for individual needs …are socially integrated and not isolated …are making a positive contribution and experiencing fulfilment …feel secure and safe …feel free from discrimination …feel financially secure …are in good health in mind and body …have dignity, choice and control throughout their life

Cost Savings and Benefits Engaging people with services and information Social inclusion and reducing isolation Getting people to appointments Supporting therapeutic activities Exercise and physical activity Opportunistic savings Soft evidence suggests c. £600,000 for 14 projects Evidence from other trusted sources and research programmes The need for a long-term study vs. gut feeling on quick wins!
