1 Preparing Your School’s FCAT SSS Documents For Return

2 ___1. Verify that all distributed secure materials have been returned. Notify the district coordinator immediately if any secure materials are missing and complete the necessary investigation. ___ 2. Make copies of the records of required administration information collected by test administrators and file the copies. (Page 24 of the manual) ___ 3. Verify that your SSS FCAT Security Log has been completed correctly. Make a copy for your files. ___ 4. Remove stray SSS math reference sheets or science SSS reference sheets/periodic tables from the test books and answer documents. ___ 5. Verify that each student has completed the required information (Student Name, District/School) in the upper right corner of the student grid sheet, even if a label has been affixed in the lower left corner. Separate the SSS test materials into stacks as follows: Your School’s SSS Answer Documents By Grade Level and document type Home Education Program SSS Answer Documents (49-9998) By Grade Level and document type McKay Scholarship Program - Private Schools (49-3518) By Grade Level and document type CTC Scholarship Program SSS Answer Documents (97-9999) Florida K-8 Virtual School Program SSS Answer Documents Connections Academy (78-0700) FL Virtual Academy (79-0801) By Grade Level and document type As You Receive SSS TEST MATERIALS From The Test Administrators, Do The Following:

(continued) Separate the SSS test materials into stacks as follows: All NOT TO BE SCORED documents which includes: used and unused test books unused non pre identified and pre identified answer documents defective documents answer documents with all DNS bubbles gridded DNS ___ 7. If a document is NOT TO BE SCORED, verify that BOTH DNS bubbles have been gridded. If a DNS bubble has been gridded by mistake, verify that the DNS bubble has been erased and the corresponding UNDO bubble has been gridded and place the document in the appropriate stack of TO BE SCORED materials.

5 Preparing Your School’s TO BE SCORED Answer Documents For Return ___8. Complete a separate SSS DOCUMENT COUNT FORM for each grade level tested. Page 229 of the manual Verify that you have included all documents from make-up sessions before you complete the forms. DO NOT combine documents for more than one document type/grade level under the same SSS DOCUMENT COUNT FORM. The SSS Count Form is scannable and must not be photocopied. Place each completed SSS Count Form on top of the first stack of TO BE SCORED answer documents.

6 Complete The SSS ANSWER DOCUMENT RETURN SUMMARY __9. Complete the SSS Answer Document RETURN SUMMARY as instructed below: Complete only one SSS ANSWER DOCUMENT RETURN SUMMARY for students enrolled in your school. There are separate SSS ANSWER DOCUMENT RETURN SUMMARY Forms for Home Education, McKay Scholarship Program and CTC Scholarship Programs. Transfer the information from each SSS DOCUMENT COUNT FORM to the SSS ANSWER DOCUMENT RETURN SUMMARY, making sure that the Number of TO BE SCORED Documents Returned for each grade level and document type is correct. Verify that the number of documents returned and the number of documents indicated on the SSS DOCUMENT COUNT FORMS match the entries on the SSS ANSWER DOCUMENT RETURN SUMMARY. Maintain a copy of the completed document for your records.

Complete the Number of Answer Documents Returned The following information has been pre identified on all Summary Forms: District Name and Number School Name and Number Grade Level and Subject Tested

8 Complete SSS Paper Bands __ 11. Documents for each grade level and document type must be paper banded separately. You may use more than one SSS Paper Band per grade level and document type, if necessary. Follow these steps for each paper band you use: Grid the appropriate grade level and document type. Write the school name and district/school number at the bottom of the SSS Paper Band. Count the number of TO BE SCORED documents that will be banded in each SSS paper band. Write that number under Number of To Be Scored Documents Contained in This SSS Paper Band. Under Number of Banded Stacks per Document Count Form, number the SSS Paper Bands 1 of n,2 of n, etc., where n is the school’s total number of banded stacks per SSS Document Count Form. Place an SSS paper band around each stack of TO BE SCORED answer documents. The SSS Document Count Form must be under the paper band on top of the stack labeled 1 of n. Seal the SSS Paper Band and be sure that it securely holds the documents. Do not use staples, paper clips, or tape.

10 Preparing McKay Scholarship Program (Public Schools) (Public Schools) TO BE SCORED Documents For Return Documents for McKay Scholarship Program students enrolled in Public Schools will be processed by using the number for the school in which they are enrolled. Scores for these students will be returned to your school. TO BE SCORED materials for McKay Scholarship students enrolled in your school should be returned with your school’s TO BE SCORED materials.

Scores for these students will be returned to the District Office. Results will not be aggregated with district or school data.

A separate Count Form must be completed for each grade level and document type. Do not combine documents for more than one grade level/document type under the same SSS DOCUMENT COUNT FORM. Write Home Education in the space labeled School Name instead of your school name. In the space labeled District/School Number, write: 49-9998 for Home Education Students Place each completed SSS Document Count Form on top of the appropriate stack of TO BE SCORED Home Education Program documents.

Scores for these students will be returned to the FDOE Office of Independent Education and Parental Choice for distribution, and results will not be aggregated with your district or school data.

21 Completing The SSS Document Count Forms For Corporate Tax Credit (CTC) Scholarship Program ___18. Complete the SSS DOCUMENT COUNT FORM by following the directions on the form. A separate Count Form must be completed for each grade level and document type. Do not combine documents for more than one grade level/document type under the same SSS DOCUMENT COUNT FORM. Write CTC in the space labeled District Name instead of your district name. Leave the School Name blank. In the space labeled District/School Number, write: 97-9999 for CTC Scholarship Program Place each completed SSS Document Count Form on top of the appropriate stack of TO BE SCORED CTC Scholarship Program documents.

26 Grades 5, 8, & 11 TO BE SCORED Answer Documents PINK Labels - Science – Due March 28, 2008 NOT TO BE SCORED Materials YELLOW Labels – Due April 9, 2008 District Coordinator Box - Create your own Label Due the week of April 21-25, 2008 It is your responsibility to package the materials correctly for processing and score reporting. Mispacked materials delay the scoring and reporting of student results.

27 Calibration Schools Reading, Math, & Science BLUE If your school has been identified as a Calibration School, you will receive BLUE labels in your return materials kit. Follow these steps for preparing your materials for return to the District Coordinator: _Place the SSS materials in the boxes according to the diagram on page 6 of the Spring 2008 FCAT SSS Materials Packaging and Return Instructions. You may place materials for more than one grade level in a box. _Place the SSS Answer Document Return Summary for Calibration Materials on top of the documents in the first box of TO BE SCORED calibration materials. Seal the box(es). BLUE _Place the BLUE label anywhere on the top of each box. Make sure that packing tape does not cover the label. BLUE _Return the BLUE labeled box to the District Coordinator.

33 Sealing The Boxes Points To Remember 1. Make sure the packing tape does not cover the label. 2. Labels are scannable and should not be photocopied. 3. Do not place labels over the meeting point of the two flaps.

34 Preparing LARGE PRINT and BRAILLE Materials For Return WHITE Label Complete the following as soon as testing has ended: _Verify that your SSS FCAT Security Log has been completed correctly. _Verify that each student grid sheet has been completed correctly or has a student label affixed. _Ensure that the student’s name and district/school number appear in the appropriate locations. _If the test is TO BE SCORED, verify that DNS bubble has not been gridded. If a test is NOT TO BE SCORED, verify that the corresponding DNS bubble has been gridded.

Place the following documents for each student tested in a separate SSS LARGE PRINT AND BRAILLE Materials RETURN ENVELOPE: Large Print Competed SSS Document Count Form Large Print SSS Test and Answer Book Or Large Print SSS Test Book Large Print SSS Answer Book Braille Competed SSS Document Count Form Regular Print SSS Answer Document Braille SSS Answer Sheets containing student responses (separated by subject) Braille SSS Test Book Regular Print SSS Test Book

37 Complete The LARGE PRINT And BRAILLE Materials Return Envelope _Complete an SSS Large Print and Braille Materials Return Envelope for each student according to the instructions on the envelope. You may place doucments for a student in more than one envelope. Seal the envelope. _Complete the SSS Large Print & Braille Answer Doucment Return Summary Form according to the directions on the form, and place it on top of the Large Print & Braille Materials Return Envelopes in the first box of Large Print & Braille materials. _Place the appropriate Large Print & Braille test materials into the boxes in which they arrived, according to the diagram on page 273 of the manual. _Return the completed SSS Large Print and Braille Materials Return Envelopes and all other materials in the original boxes in which they arrived, according to the diagram on page 273 of the manual. _ Seal the box(es). __Place a WHITE label anywhere on the top of each box. Labels should not be photocopied.
