1 TPPP Preceptor Workshop Developing Nursing Skills specific to Aged Care

2 On the next slides are some areas that have been identified as being challenges to the new graduate working in Aged Care. As the result of a workshop with experienced RNs in the field these areas were discussed and key pointers were identified. Developing Nursing Skills specific to Aged Care

3 Making medical decisions Know resident – familiarity Check palliative care order Medical history Know advance directives Forms are updated Treatable conditions not ignored because of palliative care Know medical officer – support doctor to write down treatment plan

4 Making medical decisions Familiarity with family wishes – update family often – builds relationship Knowing hospital avoidance strategies as well Promoting use of “gut” feeling – will be supported, use documentation to support this decision Knowing sometimes that you don’t have resources to manage Using PRN pain relief – discussion around use of morphine

5 Dealing with accountability/responsibility/ delegation Need to delegate – job description What to delegate – who to delegate to – Eg. Change in allocation – bring back to resident Complaints about delegation – give reason/explanation as to why delegation occurring Ask for volunteers Pick the most suitable for task Delegate at meeting not with list

6 Working with GP Familiarise graduate with likes and dislikes of GP

7 Managing resident behaviour Behaviour management awareness Expectations around dementia in clinical areas Dementia training – promotion by preceptor Co-existence: Awareness of supportive strategies for lucid residents- early strategies identified Training re PRN medication on behaviour (pharmacological)

8 Knowledge of basic screening process for behaviour – u/a etc to be able to report to doctor (have it ready) Tap into dementia specialist within organisation Managing resident behaviour

9 Working with families Expect difficulty Do not take it personally Document complaints and follow up Explore issue – stakeholders – issues – intervene – follow up Refer with senior staff Constantly update

10 Know when to call case conferences eg Requests for additional services, medical issues that need to be resolved Conflicting expectations Working with families

11 Staff conflicts Know all sides Using mediation person Common goal – quality client goal Resources available Team approach Difficulty in approaching someone older – awareness Awareness of difficulty with moving up roles eg PCW-EN-RN

12 Time will help to gain acceptance in job role Framework for resolving conflict – Confidence – Acknowledge, 2 sides, aware I’m aware, resolve here and now or manager, come up with solutions, aware of process of unresolved conflict – Get parties to provide information about issue – Find out what ACTUAL issue is – through discussion – listening, acknowledge Staff conflicts
