2 What is transition? Transition is the period of time when young people move from being a child to an adult. It can be a difficult time for people with disabilities. We want to make sure that transition in Wolverhampton is as smooth and successful as possible.

3 To support good transition in Wolverhampton for young people aged 14-25 with additional needs 1.Working with multi agency partners 2.Supporting and co-ordinating the Transition Support Group meetings 3.Helping to develop and launch the Transition protocol and make sure it is working

5 Was launched in March 2011 Agencies work together Tells everyone what we all do and when we should do it We start at age 13 and carry on until someone is 25 years old Transition Protocol

8 1.I don’t need to worry about transition until my child is in Year 11 2.Transition is discussed within the Annual Review of my child’s Statement 3.Parents and carers don’t need to attend transition reviews 4.Parent’s and carers should arrange their child’s transition review 5.Transition planning is only about education and employment 6.Something called a Transition Plan will be drawn up by your child’s school 7.Parents/carers and young people should be given a copy of the plan and the opportunity to contribute to it only if they ask 8.All young people with a social worker in children’s services will receive one in adults 9.Statements will follow a young person if they go into further education 10.All young people with a Statement will be allocated a Connexions Personal Adviser (PA) 11.Connexions PAs will continue to support young people until they are 25 12.The Local Authority has a legal duty to secure education or training for young people over 16 and under 25

9 FALSE! If your child has a Statement of Special Educational Needs, transition planning will begin when they are in Year 9. This may seem early but, in order to ensure your child’s transition is smooth and successful, forward planning is essential. Your son/daughter will be making choices that ultimately impact on what they do beyond Year 11. Planning is ongoing and any decisions made will be reviewed regularly as your child moves through school to ensure their wishes and aspirations are taken into account.

10 True! The Year 9 Annual Review will be called a Transition Review. In addition to discussing your child’s Statement of Special Educational Needs, the meeting will focus on drawing up a plan for your child’s future. This meeting; Should fully involve young people Should be person centred

For those young people who are at School Action Plus, a section 139a Assessment may be carried out if the Connexions Personal Adviser feels it is needed.
