2 2 After Test Administration Overview of Materials Return Process The descriptions and steps below and on the following slides provide a general overview of the materials return process. Detailed instructions are included in the 2014–2015 FSA Paper-Based Materials Return Instructions. School assessment coordinators will complete the following steps before returning materials to district assessment coordinators: 1.Receive all secure materials from test administrators. 2.Ensure all test and answer books have the appropriate labels applied:  PreID Labels should be used on TO BE SCORED test materials whenever possible. On-Demand PreID Labels may be printed for any students for whom PreID Labels were not included in the shipment of test materials (e.g., students who enrolled after the PreID upload.)

3 3 After Test Administration Overview of Materials Return Process (cont.)  District/School Labels should be used on TO BE SCORED test materials if PreID Labels are not available and cannot be printed. Student demographic information must also be gridded on documents with District/School Labels applied, and student information must be entered in TIDE.  DO NOT PROCESS Labels should be applied to all used NOT TO BE SCORED materials, including used defective documents and documents from which student responses have been transcribed into another test and answer book. 3.Separate TO BE SCORED and NOT TO BE SCORED materials.

4 4 After Test Administration Overview of Materials Return Process (cont.) 4.Place all TO BE SCORED materials in the plastic return bags included in your test materials.  Separate TO BE SCORED regular print materials from TO BE SCORED special documents (large print, braille, one-item-per- page).  It is not necessary to separate materials by grade level, subject, special program, etc.; however, if your district assessment coordinator provides instructions for sorting materials, please follow those directions.  Document Count Forms are NOT provided, and therefore are not required for processing FSA TO BE SCORED materials.

7 Before Test Administration 7 PreID Labels Similar to previous administrations labels. Printed and provided based off of PreID upload. If applied to a document, student information does not need to be gridded. If additional PreID Labels are required, On-Demand PreID Labels may be printed at the local level. Blank label stock will be provided in initial shipments and also available for additional orders. Sample PreID Label

8 The preidentification information provided by districts is used to print PreID Labels and rosters. If the district supplied class code information, the labels are sorted by class code; otherwise, they are sorted alphabetically by grade level. School staff may verify and apply labels and/or complete grid sheets no sooner than one week prior to testing. On-Demand PreID Labels Districts and schools will receive blank labels to create On-Demand PreID Labels in TIDE for students who need them. Additional blank labels may be ordered through TIDE, if needed. Preidentified Student Labels 8

9 Affix PreID Labels Please adhere to the following policies when preparing student materials: Affix labels in the box on the student grid sheet that appears below the statement, “ALIGN TOP OF LABEL HERE.” Please note that this box contains a barcode used for scanning unused documents, which will be covered by the PreID Label on TO BE SCORED documents. Only the school assessment coordinator and other certified school personnel may open the shrink-wrapped packages and affix labels. Sealed test and answer books must not be opened. Do not use student labels from previous administrations. Students and non-certified school personnel are not permitted to assist in preparing secure materials before testing or in organizing and returning materials after testing. The Test Materials Chain of Custody Form must be maintained at all times. All secure materials must be returned to locked storage and remain there until testing. Preidentified Student Labels 9

10 Affix PreID Labels (cont.) Follow these steps when verifying student information and affixing a PreID Label to a student grid sheet: 1.Verify that the student is still enrolled and that the student will be tested. Labels for students not testing should be destroyed in a secure manner because they contain student information. 2.Verify the information on the PreID roster against the school’s student information database. Because the complete Social Security Number is not printed on the label, the PreID roster must be used to verify student information. It is not necessary to verify the labels against the roster because the same data file was used to create both. 3.If any information other than the student ID number (Social Security Number or Florida Student Number) is blank or incorrect on the PreID Label, the information should be entered in TIDE, but the label may still be used. If the student ID number is INCORRECT, a new student record must be created in TIDE and an On-Demand PreID Label should be printed. Preidentified Student Labels 10

11 Affix PreID Labels (cont.) Follow these steps when verifying student information and affixing a PreID Label to a student grid sheet (cont.): 4.In order for a document to be properly scanned, carefully place the label on the grid sheet in the box that appears below the statement, “ALIGN TOP OF LABEL HERE.” Ensure the label is not wrinkled or applied unevenly. 5.Do not remove labels once they have been affixed; however, if information on a label is incorrect, a new label may be printed and applied over the incorrect label. Note: if an On-Demand PreID Label cannot be used, apply a District/School Label in its place and hand-grid the student demographic information and update the student’s information in TIDE. 12 AfterTest Administration 12 District/School Labels Details district/school for reporting purposes. District/School bubbles are no longer on demographic page. Applied to any answer documents with no PreID Label attached. Demographic information must be bubbled on the answer document if a District/School Label is used. Provided in initial shipments and also available for additional orders. Sample District/School Label Students with hand-gridded documents must be entered into TIDE. NOTE: If Pre-id or District/school label is applied it is considered USED, even if blank. Replaces DNS bubble for NOT TO BE SCORED documents. Document is accounted for but not processed and reported for student accountability. Placed over Pre-id or District/School label Does not have to be applied to unused documents. Provided in the initial shipments and available for additional orders.

14 Returning Test Materials 14 DRC Return Bags Place TO BE SCORED materials in the bag before boxing them for return. Reduces process time by reducing humidity levels. Assists in the prevention of missing materials if boxes break open upon return. Provided in the initial shipments and available for additional orders.

15 Returning Test Materials 15 Return Labels Use the following chart, found on page 7 of the Materials Return Instructions to ensure you apply the appropriate color label to each box: * *Schools that receive blue labels will not receive green (ELA Writing) or orange (ELA Reading and Mathematics) return labels. Blue return labels should be applied to TO BE SCORED materials, and materials should be returned as instructed in the 2014–2015 FSA Paper-Based Materials Return Instructions.
