6 6 New programme Developed from inter-agency discussions and evidence: –Addresses stated health needs of young people, families and schools –Takes account of priorities within Single Outcome Agreement and CHP plan (HEAT targets etc) (workforce planning) –Based on GIRFEC principles / inclusive of disability –Seeks to address causes rather than symptoms –Integrated approach –developing core team/wider circles of influence –Child and Family at the Centre /early interventions –Sustainability is key –
