8 17-Sep-15Saskatchewan Falls Collaborative8 Changes Tested Appropriate footwear Check the special needs unit and discover that residents (out of 12) where wearing the appropriate footwear

9 Nursing staff reading the FHHR policy Initial findings- 42% staff read policy. Took actions by using calendars and talking to staff in person to read the policy, these actions led up to the 92% of all staff knowing/reading the FHHR policy. Changes Tested

10 We piloted a q-30min check of the residents on special needs wing. Duration was June 24 th - July 24 th, we discovered this was the highest month of falls. Staff did not like the added documentation. We will re-look at this project. Changes Tested

11 17-Sep-15Saskatchewan Falls Collaborative11 Lessons Learned It is difficult to work as a team when all members work rotating shift work. The more that staff are involved and educated, the more progress we make. We need to have a look at the results of what we have already done to determine what we do next. We appreciate being a part of this group.
