3 Every day in our Health District 6 residents die: – 1 of the 6 is tobacco related – 2 people die < 75 years – 1-2 from potentially preventable conditions 10 babies are born – 2 are Aboriginal – 1 has a low birth weight – 1 has a teenage mother 60 are admitted to a private hospital 200 are admitted to a public hospital – 155 are <75 years old – 21 of them are children (5 Aboriginal) – 11 are potentially avoidable admissions 587 people present to an emergency department – 73% conditions that are not urgent – 7% serious illnesses/injuries requiring urgent treatment 2,800 people consult their GP

4 Health Needs Assessment The interventions likely to provide the greatest population health outcome gains: 1.Smoking prevention and cessation 2.Nutrition and physical activity interventions 3.Diabetes prevention and management 4.Well child care during the first 1,000 days of life 5.Mental health - strengthening community care and support

7 Integrated Care Strategy 3 year strategy with $15M investment as the lead Rural LHD to Transform existing services into an integrated Western NSW system of care that is tailored to the needs of our rural and remote communities, improves access to care and health outcomes, focuses on closing the Aboriginal health gap

8 eHealth Investment $11M investment over 3 years to rapidly accelerate eHealth capability Significant bandwidth upgrades Improvements to local technology Access to shared clinical summary record (HealtheNet) Rollout of eHealth systems to every site: EMR2 Community Health and Outpatient Care system (CHOC) Electronic Medications Management A wide range of corporate & support systems Telehealth as a cornerpiece of rural service delivery
