Wellness Services offered: Education in the following areas: Smoking Cessation, Diabetes, Hypertension, Obesity, Nutrition, Hyperlipedemia, Illness Self Management Use of Peers: Advisory Committee EHR Vendor: Netsmart Technologies, Inc. – CMHC/MIS

Staffing: Physician’s Assistant (Contracted FQHC) Director 2.2 Registered Nurses PHD level pharmacy students (Affiliation)

Finance/Sustainability FQHC partner to make formal application to the Federal Government to incorporate Coastal’s Primary Care Behavioral Integration program as an official FQHC site. This FQHC status will allow for billing which will support sustainability for integrated services. This approach will also allow for dual enrollment of the patient into both systems

Health Information Technology (HIT) Developed a computerized NOMS Data Collection Services Tool Scanning of paper record into a document management system for electronic availability as an interim step to EHR implementation. Hired a PHD psychologist with 13 years experience in an EHR software development company as the HIT grant Director. The revision of the Front Desk work flow to ensure accurate data collection with each visit. The development of a new work flow process for the collection, monitoring, and accountability of Enrollment and Reassessment data for the TRAC system.

Other Wellness Established a partnership with Lake Erie College of Medicine, School of Pharmacy to provide two students in the clinic for a total of 80 hours per week. Access to pharmacy consultations for providers and patients Developed a “Ask a Pharmacy Intern” call line Every student develops and presents a health related presentation to patients and staff Students develop educational handouts on medication classifications Students assist in collection of data, assessment and screening.

Continue recruitment and enrollment Increase peer involvement Promote increased patient centered care Compare and contrast current vs. previous health status Work towards the completion of an EHR.
