2 Fifth Annual LBNL Safety Culture Survey Runs now until November 9 th – next Monday Results will be posted online in December Results will be presented at Monday Morning Meeting Anonymous, but if you include your name in a comment, you lose anonymity Valuable source of feedback –on employee perceptions of safety at LBNL including the role that managers and supervisors play –the effectiveness of ES&H programs –the receptiveness to reporting events or injuries and other areas

4 FY09 Safety Culture Questionnaire 48 questions ~ 10 minutes Asked to identify –Division, employment status, classification (supervisor, work lead, non-managerial employee) Can add comments Categories of questions –Personal experience with safety –Line management commitment to safety –Work group safety –Ergo –Environmental –Policies and procedures –Information flow EHS to worker Worker to EHS
